The type of personal information we collect
We currently collect and process the following information:
· Personal identifiers, contacts and characteristics: name, address, age, telephone number, email address
· Via the website we may also collect IP address, device number, plus other statistical data that is captured by cookies
· Health details (1): any particular conditions or illnesses that you share with us
· Health details (2): any covid related information that you share with us
· Health details (3): the name of your GP or practice and the name and contact details as an emergency contact
How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you for one of the following reasons:
· For membership of "The Diet and Dance Club" and to provide you with the services that you are entitled to book as part of that membership
· To provide you with details, updates or changes of those services that you have booked or may want to book
· To contact a doctor or any person in the event of an emergency
· To inform us of any particular condition or illness that we including our subcontractors need to be aware of before we provide you with any service
· To ascertain any interest in any additional services that we may look to offer
We may share this information with:
· Any subcontractors we use to for the provision of the services, for example we may share some health information you provide to the nutritionist and dance instructor prior to providing you with the service.
· With any payment provider to process any payment and/or answer any queries.
· Any regulatory body such as a dance body, ICO, or charity commission or HMRC to process any donations.
· The police where we believe that a crime has been committed or may be committed.
· With any medical practitioner like a paramedic or GP in the event of health emergency or incident.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
· Your consent. You are able to remove your consent at any time. You can do this by contacting us on info@dietanddance.com
· We have a contractual obligation.
· We have a legal obligation.
· We have a vital interest.
· We have a legitimate interest.
How we store your personal information
All physical or paper records will be scanned and then stored safely at our registered offices on local servers with appropriate off location backups. Once scanned all paper records will be destroyed.
Your electronic records submitted via the website are stored on servers provided by our website provider GoDaddy and their servers are located in Amsterdam, Netherlands.
We will keep:
· your personal data for as long as you are a member and for any period that we are required thereafter as matter of company and tax law (normally no more than 7 years);
· your health data only for as long as you are a member plus any additional period that may be in force for Covid related issues
We will then dispose your information by destroying any paper records or deleting any electronic records.

Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
